Notice to Consumers: Texas Mental Health Services
In accordance with Texas law (House Bill 4224 and Section 181.105 of the Texas Health and Safety Code), the following information is provided to help consumers understand their rights and available resources.
Requesting your Healthcare Records
Current and former clients have the right to request copies of their mental health records maintained by Evergreen Professional Counseling. Records will be provided in accordance with Texas law and abiding by applicable privacy regulations.
To request your records, please submit a written request to Evergreen Professional Counseling via email and include full name, description of records being requested and dates of treatment as well as preferred method to receive records.
